Approach to Proteinuria

Definition and classification of benign proteinuria

Type  Criteria
General
  • Previously defined as > 150 mg/day in adults
  • Current definition based on albuminuria
    • Moderately increased albuminuria (30-300 mg/day)
    • Overt (severely increased) proteinuria (> 300 mg/day)
  • Mild proteinuria
    • > 150 mg/day but < 1.5 g/day
  • Heavy proteinuria
    • > 3.5 g/day
Isolated proteinuria
  • No abnormalities in urinary sediment
  • No decrease in glomerular filtration rate (GFR)
  • No hypertension or diabetes
Transient proteinuria  (< 1 g/day)
  • Possible etiologies
    • Fever
    • Heavy exercise
    • Vasopressor infusion
    • Albumin infusion
Orthostatic proteinuria  (Usually < 1 g/day)
  • Increased protein excretion in upright position
  • Normal protein excretion in supine position
